Vismodegib

Vismodegib is the first FDA-approved Hedgehog (Hh) pathway inhibitor, binding allosterically to Smoothened (SMO) and blocking downstream GLI1/2 transcription factor activation. It is highly effective in basal cell carcinoma (BCC) driven by PTCH1 loss or SMO gain-of-function mutations. FDA-approved for locally advanced/metastatic BCC; also studied in SMO-mutant medulloblastoma.
